Engineering made some holes, and maintenance sealed them up. I don't think they read the instructions.
Reminds me of the dairyman from China who observed the cheese making process in Switzerland and reported home that he saw what they were doing wrong, and that he could produce the cheese without the holes in it.

I would have thought that a problem like that would have shown up in testing before Boeing delivered the updated unit to JAL.

I'm always amazed at the slack built into scheduling - that they could wheel out another plane capable and ready for a long-haul flight, and only delay by 1 hour. I always thought that the planes were worked as hard as possible. There must be a lot of jets out there on 'ready alert' !
Yea, I'm more or less human and I still get the occasional 'hey man'....
