The question that seems obvious to me is how was the "mandatory" testing circumvented? And why did it take so long to discover?

Bribe the supervisory system.

Yes, you're probably right. There's uncaring, undisciplined, unscrupulous people everywhere, and as the world population grows these type people find it easier to operate.
